Post-Master’s degree applicants who wish the Graduate Diploma in primary health care nurse practitioner (PHCNP). Primary Health Care Nurse Practitioners (PHC NPs) are registered nurses, who are specialists in primary health care, who provide accessible, comprehensive and effective care to clients of all ages. They are experienced nurses with additional nursing education which enables them to provide individuals, families, groups and communities with health services in health promotion, disease and injury prevention, cure, rehabilitation and support. The Council of Ontario University Programs in Nursing (COUPN) established the Ontario Primary Health Care Nurse Practitioner (PHC NP) Programme in 1995. A consortium of nine Ontario universities offers the program. Graduates are eligible to write the Canadian Nurse Practitioner Exam Examination: Family/All Ages to qualify as an NP-PHC. |

Application Notes : | Applicants whose native language is not English will be required to submit an official copy of their TOEFL score, or other evidence of competency in English is required. A minimum TOEFL (iBT) score of 92 (580 on the paper-based TOEFL test or 237 on the computer-based TOEFL test) is needed for the Faculty of Health Sciences. Students taking the IELTS are required to achieve a minimum score of 7. Equivalent scores on other recognized tests may also be considered. Each of these scores are only valid for 2 years. Students who have completed an Academic ESL program through Canadian academic institutions may petition to have this considered in lieu of TOEFL. Applicants may be exempted from this requirement if they have completed a university degree at which English is the language of instruction. |

Applicants are required to submit two academic and two clinical references, a confidential letter from two persons who have known them in a professional context, a Curriculum Vitae (including a list of publications and funded research grants), and a personal essay responding to four questions related to suitability for the PHCNP program.
